Initial Comments:
An offsite follow-up survey conducted on September 6, 2023 found that ECHO Hospice of Pennsylvania, LLC had corrected the deficiencies cited under the requirements of 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a State Re-Licensure Survey completed on July 28, 2023.
